Approval Forms

Please submit all forms to the REB Coordinator (Sue Funk, Upper SNC).  If you are unsure as to which form to use, please contact Sue at sue.funk@twu.ca or 604-513-2142.

Make sure you allow enough time for your application to be reviewed. For faculty research, thesis research and MAL projects, an average application that qualifies for expedited review (two reviewers) should take approx. four to six weeks (from when it is received by the REB) for the review process.  Applications required full REB review should take approx. six to eight weeks.  For small class projects, an average application should take three weeks (from when it is received by the REB) for the review process.  There may be additional time needed for all types of applications if modifications are required.  Most applications will require modifications.

Do not start research with human subjects (done directly or through analysis of existing data) until you have received an email from the REB stating that your project has been approved. This includes the recruitment of subjects.

It is highly recommended that you complete this tutorial before filling out an application for Faculty, Graduate Thesis, MAL Projects or Undergraduate Thesis research.  (There are sometimes problems printing out the certificate at the end.  Print out the last page where it says "Congratulations ..." as proof that you have completed the tutorial in case you encounter this problem.) Please note: If after being reviewed your application requires major revisions, you will be required to complete the tutorial.

Professor Forms

The following forms are to be used for course research projects where all students complete exactly the same assignment. The form is to be filled out by the professor teaching the course.

Student Forms

The following forms are to be used for course research projects where the student or groups of students complete individualized research projects. The form should be filled out by the students with the professor's assistance.
