
If you are interested in applying to Access Ottawa here is a list of the following steps that you will need to complete. Please note that applications cannot be submitted online.
There are two ways of completing the application form. 1) You can type out the answers to your application and print it off, or 2) You can print off a blank application and fill it out by hand as hand written applications are also accepted. Applications must be sent in the mail as originals are required. Preference is given to applications received before March 1, 2011. Thank you!
Step 1- Fill out Section A- Student Application Form. Print off the application form. Fill out your information, sign your application at the bottom of the page. Don't forget to attach your photo as it will be used for your student ID tag for Access Ottawa.
Step 2- Fill out Section B- Applicant Consent Form. Have your parent or guardian read over this section and sign it.
Step 3- For Section C- Applicant Reference Form. You will need a teacher or pastor to fill out this form for you. This form is to be submitted separately by your reference. They can either fax or mail this part of the application. Please note that we wait until complete application is received before reviewing it.
Step 4- Deposit. Don't forget to attach your $100 deposit. The deposit goes towards your tuition if you are accepted, and will be refunded if you are not accepted. The remaining amount will be due upon acceptance. Cheques are to be made payable to the "Laurentian Leadership Centre".
Step 5- Mail Application Form. Look over your application form and make sure all sections are completed, signed, and deposit is attached. Please mail your application to: Access Ottawa, c/o Laurentian Leadership Centre, 252 Metcalfe Street, Ottawa ON, K2P 1R3
